For making a food diary you'll have to write down what you eat for one week. Also mention the amount and the time. Record this also for everything that you drink. Give a general description of your eating habits and tell me which things you really like or dislike. Also mention what you want to achieve by adjusting your diet. When you use medication or you suffer from any ailments it's important to mention this as well.
